We share your concern about the cost of medical care and
believe health care should be affordable and accessible. The Duluth Family Practice Center's fees are standard
for a primary care clinic in Duluth and the surrounding areas.
Information about our charges are available upon request.
Should you have special circumstances that make payment difficult, please
let us know in advance so we can arrange for a mutually acceptable payment
program.
Your Billing Statement
You will receive a statement from our Business Office
listing the services provided at your visit, charges submitted, and payment
received from your insurance carrier. Often times there is a patient
responsibility associated with payment of medical services that is not
covered by your insurance carrier. Your statement will indicate
payments received by the insurance carrier and the balance due that is your
responsibility.
If you do not have insurance, your account will be considered self-pay.
You are responsible for payment of medical services provided on the day of
your appointment.
We recognize that sometimes total charges can be
significant and we offer payment arrangements. If you need to make
payment arrangements, please talk to one of our patient account
representatives.

As a service to our patients, we will
submit claims to your insurance carrier for medical services provided at
the Duluth Family Practice Center. Please be sure to bring your
insurance or other payer information with you to each appointment.

There are many types of insurance carriers and there can be significant
differences between plans. It is important that you are knowledgeable
about the specifics of your plan and areas of coverage provided to you.
Our patient account representatives will do all they can to assist you with
any insurance issues or questions. However, please realize it is your
responsibility to understand the coverage pertaining to your specific
insurance carrier.

Referrals
We will coordinate and process your referrals for needed specialty services as
specified by your primary care provider. You can help by letting us know when
your insurance company or health plan requires a referral or prior authorization
for specific procedures. If you have questions about referrals, call our
